Support Beyond Clinical Care
This service provides vital support for clients who may struggle with life adjustments due to illness, aging, or disability. The medical social worker helps create an environment where care plans are more achievable by identifying and mitigating psychosocial barriers. They ensure open communication with healthcare team members to align social support with the patient’s clinical goals. Their ability to identify family dynamics, living situations, or financial constraints allows for more customized care strategies. By collaborating with healthcare providers, medical social workers bridge gaps in care that may otherwise go unnoticed.
